Register Your Wrestler

Wrestling is a physical, contact sport and carries an inherent risk of injury. By signing below, I give permission for the wrestler named above to participate in Calaveras Youth Wrestling practices, tournaments, and related activities, and I release the club, its coaches, and volunteers from liability for injuries sustained during participation, except in cases of gross negligence.

In the event that my child becomes ill or injured while participating in Calaveras Youth Wrestling activities, practices, tournaments, travel, or related events, and I cannot be contacted immediately, I authorize Calaveras Youth Wrestling coaches, volunteers, and representatives to obtain emergency medical treatment for my child as deemed necessary by licensed medical personnel.

I understand that reasonable efforts will be made to contact me prior to treatment whenever possible. I agree to assume responsibility for any medical expenses incurred on behalf of my child.

I grant permission to Calaveras Youth Wrestling to photograph, videotape, and/or record my child during practices, tournaments, events, and activities. I understand that these images and recordings may be used for promotional, educational, fundraising, website, social media, and other club-related purposes without compensation.

I understand that participation in wrestling involves inherent risks, including but not limited to falls, collisions, physical contact, sprains, strains, fractures, concussions, serious injury, permanent disability, and, in rare circumstances, death.

I voluntarily allow my child to participate in Calaveras Youth Wrestling practices, tournaments, conditioning sessions, travel, and related activities. I acknowledge and accept all risks associated with participation.

To the fullest extent permitted by law, I release, waive, discharge, and hold harmless Calaveras Youth Wrestling, its coaches, volunteers, officers, directors, sponsors, event organizers, facility owners, and affiliated organizations from any and all claims, liabilities, demands, actions, damages, or causes of action arising out of or related to my child's participation, including injuries that may occur during practices, tournaments, travel, or related activities, except where caused by gross negligence or willful misconduct.

I certify that my child is physically capable of participating in wrestling activities and that I have disclosed all known medical conditions that could affect participation.

By signing below, I acknowledge that I have read, understood, and agree to the terms of this registration form, medical authorization, photo consent, and liability release.
